Permission to Skate /NRP forms

AAA tryouts are fast approaching. AA will follow shortly after. As per OMHA rules, players must register with their home center in order to receive a permission to skate or NRP form. 

If your child plans to attend Chiefs or AA tryouts you need to do the following:
1. Complete the WLMHA 2016/17 registration form. (Registration_form_2016.pdf)
2. Write a post-dated cheque for $200 dated May 1st 
3. Contact Jeff Woods ([email protected]) and give him the registration form and cheque, he will then fill out the permission to skate form or NRP form. 

If you have any questions please don't hesitate to ask. We kindly request that you plan ahead and remember that we are volunteers with lives and families of our own outside of hockey. Please don't wait until the last minute to request a form.
