2026 Golf Tournament Registration
Please complete this registration to secure your spot for the West Lorne Minor Hockey fundraising golf tournament. If you are golfing, your dinner ticket is included. If you are NOT golfing, and only want to purchase dinner tickets, you can use this form to pre-purchase up to four (4) dinner tickets by filling in the information about up to four (4) individuals below.

Payment Information

Please indicate how you pay your registration fees. Prices are $125/golfer. If you are NOT golfing, and only paying for dinner tickets, it is $40/ticket for the sit down meal. All fees must be paid by May 1st.
